Does anyone know the name of this UI ?

Looks like KaTnui. Definitely has TKing graphix. I have that spell gem bar, too..can't recall name of it, but it's a TKing again.
You can get most of this stuff from http://www.eqinterface.com
Check out the Comlete sets for one of the Fantasy or Midieval designs (what you want is one of the Celtic flavors). Plus go to the Thomas King (ie, Tking) section and download the Necro/Skull spell gem bar.
If you post this image there at their chat/forum, they will quickly help you find this one, too, if you have problems.

Speaking of UI's: how do i install one. I found one i liked on eqtoolbox and downloaded it into the EQ skins file, however, when I attempt to load it in game it doesn't show up. What am I doing wrong?

OK, first rule: never use the EQ skins load in-game.
I'll describe this from the beginning, so bear with any redundancy.
1) Download/Unzip your UI into a new folder. Call it anything you want ("Necro" perhaps), but it must sit in your UI folder (sony/eq/ui/).
2) Go to the character file back in the EQ folder. It will have a name something like this:
UI_Bubba_Cazic
Open it up using Notepad
3) Look for a the command line near the top (within the first 20 lines) that has something like "=default" in it. This is the command that points to the UI file folders you have, and is probably set to the EQ Default directory.
4) change the name of the directory to the new one you created/unzipped so that the command line ends with a "=Necro" or whatever you called it.
Basically you are manually pointing your character's UI file to point to the right folder, instead of having EQ do it for you (and it has never done a good job for me, ever).

im a completly computer illeterate, can u explain how to download a UI 100% like how/ where the UI file is and such. i seriously have no idea what anything on my dell does except how to connect to interenet and start up eq.

OK, first rule: never use the EQ skins load in-game.
Try using /loadskin <folderaname> 1
The one saves your Screen positions and such
I have used that since the start and never had an issue with it
Soulsapper, you are in for a real trip if you start using full blown UI's. I maintained a UI for a year, it was a lot of work. Everytime EQ did a patch, you were scrambling to get your UI fixed to get back online. I finally seattled on a few simple mods that never seem to get touched, like the spellbook, spell gems, and player window ( I want exp and AA displayed at all times ). Other than that, I go with the standard layout...and my life has been much happier since. I no longer break into a sweat when that patch window comes alive :)

Funny you should say that I did get the UI added however I can't tell what the hell spells I have from time to time and it does not bring up the small popup to say what it is. With that being said I am going back to the old UI format where life was simple and ....LOL

For anyone who uses UIs, and trembles in dread with each new update from Sony, there is a solution, but only if you are computer-literate and friendly digging into the UI files.
EQinterface.com always posts the necessary command changes to reflect UI updates from Sony, so you can patch your own UI files.
Again, not a solution if that type of thing doesn't make you comfortable, but for those who don't mind, it is a good way to have the code monkeys at EQinterface tell you what needs to be updated and how to do it.
I've been using alternate UIs since pre-LoY and have always managed to keep them updated and bug free - otherwise take Tumi's advice and mod only things that won't get updates, it is easier that way :)
